Thank you for your interest in volunteering with CEO Financial Literacy Academy. This form serves as a
comprehensive agreement covering volunteer participation, media release, and community service hours
acknowledgment. By signing below, you acknowledge and agree to the following terms:
1. Volunteer Agreement
Non-Employment Acknowledgement
I understand and agree that my participation with CEO Financial Literacy Academy is voluntary and does not constitute a job or offer of employment. I will not receive compensation for my services, and this opportunity is not an employment contract.
Program Overview
CEO Financial Literacy Academy is a nonprofit post-secondary educational program focused on workforce readiness through:
• Youth & Young Adult Development
• Workforce Readiness
• Personal Development
• Customer Service Skills
• Food Preparation Skills
• Resume Building & Job Placement Assistance
• Team Building, Sales, & Soft Skills Training
Educational Grants and Internships
Participation in educational grants or internships through CEO Financial Literacy Academy is for educational advancement only and does not guarantee employment.
Liability and Responsibility
I agree to perform my volunteer duties diligently and release CEO Financial Literacy Academy from any liability for injury, loss, or damage incurred during my participation.
2. Media Release (Video and Photo)
Permission to Use Likeness
I grant CEO Financial Literacy Academy permission to capture and use photographs, video recordings, or
digital images of me during my participation in Academy activities.
Media Usage
These images and videos may be used for:
• Promotional, educational, or informational purposes
• Printed or digital marketing materials
• Website content, social media platforms, or publications
• Newsletters or presentations
Duration of Permission
This permission is granted in perpetuity without expectation of compensation.
Revocation
I understand that I may revoke this permission in writing, except for materials already in production or previously released.
3. Community Service Hours Acknowledgement
Certification of Service Hours
CEO Financial Literacy Academy certifies that volunteers are eligible to receive official documentation
of community service hours completed during their participation in Academy activities.
Recording and Tracking Hours
Volunteers are responsible for accurately tracking their community service hours using provided time
logs. CEO Financial Literacy Academy will validate and sign off on documented hours, which can be
used to meet school, university, or other community service requirements.
Requesting Service Hours Documentation
Upon completion of the volunteer service, participants may request a formal letter or certificate verifying the total number of service hours.
